If you would like to submit a fallen military service member to be honored by Memories of Honor, please read the form carefully and reply to all form boxes.
ELIGIBILITY - PLEASE READ CAREFULLY
We honor all branches, all dates of death, and all loss of life either during active duty or post-service as outlined below:

Killed-in-Action

Missing-in-Action
-
Training Accidents
-
Self-infliction (PTSD, Suicide)

Any loss of life while on active duty.

Medical issues DIRECTLY RELATED TO MILITARY SERVICE (ex: Agent Orange, Gulf War Illness, Burn Pit Exposure, TBI, etc).

OTHER (Requires explaining HOW the cause of passing was related to military service in the "information about Hero" section. Omitting this information may delay or prevent approving your submission.)
As military families and veterans, we are sensitive to and understand the pain surrounding the loss of a loved one; and, although all loss of life and service to our country is significant, we must limit the scope of our honorees to stay in alignment with our core mission of making EVERY day Memorial Day for the families of the fallen.
