If you would like to submit a fallen military service member to be honored by Memories of Honor, please read the form carefully and reply to all form boxes.
ELIGIBILITY - PLEASE READ CAREFULLY
We honor all branches, all dates of death, and all loss of life, whether during active duty or post-service, as outlined below:

Self-infliction (PTSD, Suicide)

Any loss of life while on active duty

Medical issues directly related to military service (e.g., Agent Orange, Gulf War Illness, Burn Pit Exposure, TBI, etc.)

Other (This requires explaining how the cause of passing was related to military service in the "information about Hero" section. Omitting this information may delay or prevent your submission from being approved.)
As military families and veterans, we deeply understand the profound pain that comes with losing a loved one. Each life lost in service to our country holds immense significance. However, to remain true to our core mission of honoring the families of the fallen every single day, we must focus our efforts on those who have made the ultimate sacrifice, as outlined above.
By filling out this form, you are giving Memories of Honor the privilege to publicly honor the service, sacrifice, and life of your fallen hero as detailed in your submission. We will share these heartfelt stories through various channels, including radio, media, printed signs, banners, game day jerseys, and more. Additionally, from time to time, we may share family impact responses to help show the profound impact of the work we do.
